UPPER FREEHOLD TOWNSHIP, NJ (MONMOUTH)–Sgt. Jeffrey Lebron of the New Jersey State Police, Office of Public Information told MidJersey.news that Troopers responded to a motor vehicle crash on July 10, 2024 at 10:58 a.m. on County Road Route 539 south milepost 39.2 in Upper Freehold Twp., Monmouth County. Based on a preliminary investigation, an Acura sedan was traveling northbound on County Road 539. A Mack semi-trailer was traveling southbound on CR 539. In the area of milepost 39.2, the front of the Mack impacted the right side of the Acura within the southbound lane. After the impact, both vehicles ran off the road, traveled down an embankment, and hit a utility pole. As a result of the crash, the driver of the Acura, Carla Rinconrumay, a 21-year-old female from Trenton, NJ, sustained fatal injuries. The driver of the Mack, Tyler Combs, a 25-year-old male from Allentown, NJ, sustained minor injuries. The crash remains under investigation, and there is no additional information available at this time.
Hope Fire Company – Upper Freehold Township Firefighters, and Captial Health EMS responded to the scene. Monmouth County Highway Department and Fire Police from Hope Fire Company maintained a detour around the accident scene.

EAST WINDSOR, NJ (MERCER)–East Windsor Township reports that due to damage sustained during the power outage yesterday, JCP&L will temporarily disconnect service to safely make emergency repairs. Approximately 7,000 residents in the East Windsor and Hightstown area will be affected between the hours of midnight and 6:00 A.M., tonight July 10 and into the morning of July 11, 2024.

HAMILTON TOWNSHIP, NJ (MERCER)–Sgt. Charles Marchan told MidJersey.news that Troopers responded to a motor vehicle crash at 12:47 a.m. on the New Jersey Turnpike south on the inner roadway, milepost 58, Hamilton Township., Mercer County. Based on a preliminary investigation, a Ford pickup truck traveling south ran off the road to the right and struck the guardrail. A Honda Accord traveling south struck the pickup in the rear. There were non-life-threatening injuries reported on scene. The crash remains under investigation, and there is no additional information.
Robbinsville Township Fire Department, Bordentown Township Fire Department and EMS responded to the scene.

TRENTON-HAMILTON TOWNSHIP, NJ (MERCER)– At approximately 8 a.m., the State Commission of Investigation (SCI) became aware that our Executive Director Chadd W. Lackey died in a fatal multiple motor vehicle accident in Hamilton, N.J. this morning.
“We are devastated by the tragic news of Chadd’s death,” said SCI Chair Tiffany Williams Brewer. “On behalf of my fellow Commissioners and the entire SCI staff, we extend our prayers and deepest sympathies to the Lackey family and his loved ones. Our SCI family, the NJ Bar and the law enforcement community have suffered a great loss today as we embark upon a time of healing and reflection on Chadd’s legacy.”
SCI Commissioner Robert Burzichelli who has served on the Commission for a decade, praised Chadd’s leadership and the impact he made on the watchdog agency. “Chadd worked his way up through the ranks to lead the Commission. Under his watch, the SCI did tremendous work exposing wrongdoing and protecting New Jersey tax dollars,” Burzichelli said. “This is a huge loss for the citizens of the State of New Jersey and for all of us on the Commission.”
Governor Murphy said, “Tammy and I are deeply saddened by the tragic passing of New Jersey State Commission of Investigation Executive Director Chadd Lackey.
“Chadd broke barriers as SCI’s first Black executive director, working his way through the ranks over nearly 20 years of service to our state. During his time at the SCI, he took on organized crime, opioid abuse, corruption, and countless other issues. There is no doubt New Jersey is a safer and more just place because of him. Govenor Murphy said. “Our heartfelt prayers are with his family and his colleagues at the SCI during this difficult time.”
Mr. Lackey became Executive Director of the SCI in January 2020. He was the agency’s first black Executive Director. A longtime member of the Commission staff, Mr. Lackey had previously served as the agency’s Deputy Director/General Counsel. As investigative counsel he directed several of the agency’s most high-profile investigations into organized crime, including a groundbreaking inquiry that identified prescription pill abuse as the precursor to a new heroin epidemic. Prior to his employment at the Commission, Mr. Lackey was an Assistant State Attorney in Miami-Dade County, Florida where he founded and managed the Community Prosecutions Unit. A graduate of Northwood University, Mr. Lackey earned his law degree from Temple University School of Law.

Change Healthcare Offering Two Years of Free Credit Monitoring, Identity Theft Protection

TRENTON, NJ (MERCER)–Attorney General Matthew J. Platkin today is sharing consumer protection reminders and raising awareness about the availability of free credit monitoring and identity theft protection services following Change Healthcare’s unprecedented February data breach.
The February cyberattack at Change Healthcare, a unit of UnitedHealth, resulted in millions of Americans’ sensitive health and personal data being leaked onto the dark web—a hidden portion of the Internet where cyber criminals buy, sell, and track personal information.
Change Healthcare is the nation’s biggest electronic data clearinghouse and is used by tens of thousands of providers, pharmacies, and insurers to verify insurance, confirm pre-authorization of procedures or services, exchange insurance claim data, and perform other administrative tasks. The cyberattack interrupted operations for thousands of doctors’ offices, hospitals, and pharmacies nationwide, and impacted delivery of care to patients.
While the definitive number and identities of affected patients are currently unknown, Change Healthcare has publicly stated that the data breach could impact up to one-third of the American population.
Typically, when there is a data breach impacting New Jersey residents, consumers receive an individualized letter or email if their data may have been leaked. However, Change Healthcare has not yet provided individual notice to consumers.
Given the delay between the data breach and notification to those impacted, Attorney General Platkin wants every New Jersey resident to know about the breach and the credit monitoring resources that Change Healthcare is offering to consumers. All New Jersey residents who think they may have been impacted are eligible for free credit monitoring and identity theft protections for two years.
To enroll in this free credit monitoring and identity restoration service, consumers should visit www.changecybersupport.com or call 1-888-846-4705.
Other steps consumers can consider taking include:
- Placing a free credit freeze on your credit report. Identity thieves will not be able to open a new credit account in your name while the freeze is in place. You can place a credit freeze by contacting each of the three major credit bureaus:
- Placing a fraud alert on your credit report. A fraud alert tells lenders and creditors to take extra steps to verify your identity before issuing credit. You can place a fraud alert by contacting any one of the three major credit bureaus.
The dedicated website and call center will not be able to provide individuals any specific details about whether their data was compromised, but representatives can guide consumers through getting set up for the free credit monitoring and identity theft protections.
Additionally, consumers should be aware of potential warning signs that someone is using their medical information. The signs include:
- A bill from their doctor for services they did not receive;
- Errors in their Explanation of Benefits insurance statement, such as medical services they never received or prescription medications they do not take;
- A call from a debt collector about a medical debt they do not owe;
- Medical debt collection notices on their credit report that they do not recognize;
- A notice from their health insurance company indicating they have reached their benefit limit; or
- They are denied insurance coverage because their medical records show a pre-existing condition they do not have.

TRENTON, N.J. – A Monmouth County, New Jersey, man was sentenced today to 120 months in prison for illegally possessing methamphetamine for distribution and possessing a firearm as a convicted felon, U.S. Attorney Philip R. Sellinger announced.
Dywann Pugh, 44, of Asbury Park, New Jersey, previously pleaded guilty before former-U.S. District Judge Peter G. Sheridan to one count of possession with the intent to distribute 500 grams or more of a mixture and substance containing methamphetamine and one count of unlawful possession of a firearm and ammunition by a convicted felon. U.S. District Judge Zahid N. Quraishi imposed the sentence today in Trenton federal court.
According to documents filed in this case and statements made in court:
On Nov. 21, 2022, law enforcement executed search warrants at Pugh’s residence and discovered over three kilograms of methamphetamine; over three kilograms of cocaine; approximately 150 grams of fentanyl; approximately $19,000 in cash; and various drug distribution paraphernalia, including a pill press and approximately seven kilograms of cutting agents used in connection with the processing of drugs. Law enforcement also recovered a Hi-Point .380 caliber pistol loaded with six rounds of ammunition.
In addition to the prison term, Judge Quraishi sentenced Pugh to five years of supervised release. Judge Sheridan previously ordered Pugh to forfeit the firearm and ammunition that law enforcement seized.
U.S. Attorney Sellinger credited special agents of the Drug Enforcement Administration, under the direction of Special Agent in Charge Cheryl Ortiz; Homeland Security Investigations Newark, under the direction of Acting Special Agent in Charge William S. Walker, and the Neptune Township Police Department, under the direction of Chief Anthony Gualario, with the investigation leading to the sentencing.
The government is represented by Assistant U.S. Attorney Matt Belgiovine of the Criminal Division in Trenton.

HAMILTON, NJ (MERCER)–Hamilton Police reported that on July 10, 2024 at approximately 6:00 a.m., a 2020 Honda Accord, driven by a 51-year-old female from Bordentown NJ, was traveling southbound on Route 130 near Klockner Road. Upon entering the intersection with Klockner Rd, she struck a 2020 Ford Edge SUV, driven by a 55-year-old male from Voorhees NJ, which was traveling eastbound from Klockner Road. The Ford Edge SUV was also occupied by a 44-year-old female from Hamilton NJ. The occupants in both vehicles were transported to Capital Health Regional Medical Center, where the driver of the Ford Edge succumbed to his injuries. The occupant of the Ford Edge is listed in critical condition, and the driver of the Honda Accord is being treated for non-life-threatening injuries. The names of the involved individuals are being withheld pending notification to family.
The preliminary investigation appears to indicate that the driver of the Honda Accord failed to observe a red traffic signal while traveling southbound on Route 130.
Assistance at the scene was provided by the Hamilton Township Fire Division, New Jersey State Police, Emergency Medical Services and the New Jersey State Department of Transportation.
The crash is being investigated by Officers J. Galant and B. Mandelko of the Hamilton Police Traffic Unit. Any witnesses are asked to contact the Hamilton Police Traffic Unit at (609) 581-4000.

UPDATE:
UPPER FREEHOLD TOWNSHIP, NJ (MONMOUTH)–Just before 11:00 a.m. the New Jersey State Police, Hope Fire Company – Upper Freehold Township Firefighters, and EMS were dispatched to County Road 539 south of Burlington Path Road for a tractor trailer crash. Troopers, firefighters and EMS are on scene.
County Road 539 is closed in both directions between Burlington Path Road and Schoolhouse Road due to the crash. Fire Police and the Monmouth County Highway Department have detours set up in the area of the crash. Everyone is asked to avoid the area if possible.
Troopers from the New Jersey State Police Serious Crash Investigation Unit are investigating the crash.
No additional details are available at this time. The story will be updated once official information becomes available from the New Jersey State Police Public Information Unit.

EAST WINDSOR, NJ (MERCER)–Firefighters responded to the New Jersey Turnpike south bound Exit 8 ramp for a vehicle fire on July 8, 2024 around 8:44 p.m. for a vehicle fire. Firefighters from Hightstown Fire Company and Robbinsville Fire Department responded to the scene. The vehicle was well involved and quickly extinguished. No further details are available.
